Alors que plusieurs grandes études randomisées récentes ont constaté des effets cliniquement pertinents de l'acupuncture par rapport à l'absence de traitement ou des soins de routine, des essais en insu comparant acupuncture versus témoin ont objectivé peu de différences ou des différences mineures. Cela soulève la question de savoir si l'acupuncture factice possède des effets particuliers non spécifiques puissants. Nous avons cherché à enquêter sur l'ampleur des effets non spécifiques associés aux interventions d'acupuncture. Trente-sept essais avec un total de 5754 patients ont satisfait aux critères d'inclusion. Les études inclues varient fortement en ce qui concerne les patients, les interventions, les mesures des résultats, la qualité méthodologique et valeurs de l'effet signalé. Parmi les 32 études ayant objectivé des résultats, la différence moyenne des effets aléatoires entre l'acupuncture simulée et les groupes sans acupuncture a été de -0.45 (intervalle de confiance 95%, -0,57, -0,34 ; I² = 54%). Les essais avec des effets plus importants de l'acupuncture sham versus pas d'acupuncture ont signalé de plus petits effets de l'acupuncture fictive que les essais avec de plus petits effets non spécifiques (β = -0,39, P = 0,029). En conclusion, les interventions d'acupuncture factice sont souvent associés à des effets non spécifiques modérément élevés, ce qui pourrait rendre difficile la détection de petits effets spécifiques supplémentaires. Comparativement à une intervention placebo inerte, les effets associés à l'acupuncture simulée pourraient être plus grands, ce qui aurait des répercussions considérables sur la conception et l'interprétation des essais cliniques.
